Abstract: 

 We present a low-cost, energy-efficient system to remotely monitor the temperature and location of vaccines in a countrywide “cold-chain”. Our system is based on FoneAstra [11] – a low-cost, microcontroller-based, programmable device that extends capabilities of low-tier mobile phones that are commonly used in developing countries.

In the system discussed in this paper, FoneAstra is enhanced with a digital temperature sensor and integrated with a vaccine cold-box used to store vaccines in a temperature controlled environment. FoneAstra continuously monitors the temperature of the cold-box, aggregating readings over a period of time.

It uses the mobile phone to which it is coupled to send periodic SMS messages with routine temperature reports or immediate alerts if  it detects abnormal temperature conditions. Additionally, it enables location-tracking of vaccines in transit, based on the mobile phone’s cell tower-IDs.

We present results from an ongoing lab-deployment done at PATH [18], our Seattle-based partner NGO for this project. Over the next few months, we will deploy this temperature and location monitoring system for vaccine cold-chains in several countries in which PATH operates.

The client in this system, which includes a temperature sensor, FoneAstra and a mobile phone, costs $50; while the server, which includes a Netbook and a GSM modem, costs $500. We discuss how our system can scale up to enable large-scale monitoring while incurring low overhead costs.

For aid organizations, knowing what local communities and beneficiaries want and need is the key to running successful, sustainable programs. In Uganda, UNICEF is using mobile phones and broadcast media to get direct feedback from Ugandans on everything from medication access to water sanitation. The project, called uReport, allows users to sign up via a toll-free shortcode for regular SMS-based polls and messages. Citizen responses are used both in weekly radio talk shows to create discussion on community issues, and shared among UNICEF and other aid organizations to provide a better picture of how services work across Uganda.
 
Sean Blaschke, a Technology for Development specialist at UNICEF Uganda, explains that uReport gathers information from participants and informs citizens of their rights and available services. Recent polls have included questions about school dropouts, water point availability, mosquito net usage, and youth employment, all collected via SMS polls.

uReport is a UNICEF project in Uganda that sends SMS polls and messages to subscribers in order to gather feedback about communities across Uganda. The information is then used in broadcast and print media to inform citizens about their rights and available services, while also acting as a means of detecting vulnerabilities in communities.

The target audience is youth in Uganda who want to share information about their communities and start discussions at a community level about available services and programs.

What worked well? : 

The project has seen a huge number of signups (more than 28,000 registered users), and reasonably high response rates (ranging between 18% and 30%). UNICEF also found that the program is mutually beneficial between them and their partner organizations, as partner organizations can use the SMS system to directly target their members, while UNICEF can use the information collected in the polls to get a clearer picture of how services and systems are working in individual communities.

What did not work? What were the challenges?: 

Challenges include:

  • Finding ways to keep uReporters interested in the project so that they answer the polls (UNICEF is currently testing out multiple incentive programs to see how they affect response rates)
  • Finding a balance between the one-to-one contact of mobile communications and the need to share information with a large number of people (partnerships were built with eight local radio stations in different districts so that information could be broadcast regularly to non-mobile owners)

Equal Access: Creating a Community Feedback Loop with Radio and Mobile Phones

We spoke with Prairie Summer and Graham Gardner of Equal Access to learn more about that organization’s work integrating educational radio broadcasts with mobile-based tools such as SMS and IVR. As they explain, this combination has enabled them to better tailor their message to their their audience and has allowed for a unique form of interactive communication.

Equal Access creates communications strategies and outreach that address the most critical challenges affecting people in the developing world. Their work has focused on communications around issues such as women and girls' rights, democracy and governance, and education.

Abstract: 

Deploying and evaluating a new technology is a major challenge in ICTD research. Introducing new technologies can be hampered by a lack of cultural insight, poor or delayed feedback, and limited evaluation procedures, among other factors. 

This short paper offers a model for introducing technology in developing regions that mitigates these factors. We call these steps the “Huzzah method,” inspired by a quotation that rightly derides technology that is introduced from afar and poorly evaluated.

The paper also includes selected portions from other work on Tangaza, whose design, implementation, and analysis followed the Huzzah method.

Abstract: 

While mobile phones have found broad application in reporting health, financial, and environmental data, there has been little study of the possible errors incurred during mobile data collection. This paper provides the first (to our knowledge) quantitative evaluation of data entry accuracy on mobile phones in a resource-poor setting.

Via a study of 13 users in Gujarat, India, we evaluated three user interfaces: 1) electronic forms, containing numeric fields and multiple-choice menus, 2) SMS, where users enter delimited text messages according to printed cue cards, and 3) voice, where users call an operator and dictate the data in real-time. Our results indicate error rates (per datum entered) of 4.2% for electronic forms, 4.5% for SMS, and 0.45% for voice.

These results caused us to migrate our own initiative (a tuberculosis treatment program in rural India) from electronic forms to voice, in order to avoid errors on critical health data. While our study has some limitations, including varied backgrounds and training of participants, it suggests that some care is needed in deploying electronic interfaces in resource-poor settings. Further, it raises the possibility of using voice as a low-tech, high-accuracy, and cost-effective interface for mobile data collection.

Claim Mobile is a platform designed to support a project that subsidizes healthcare by reimbursing health service providers in Uganda for treatment of patients with sexually transmitted infections. As with many development projects, the Uganda Output-Based Aid (OBA) project involves a number of stakeholders: the service providers, the project implementers,the financiers, and the Ugandan government. Design of an appropriate solution requires meeting the various and conflicting requirements of all of these stakeholders. In this paper we detail the rapid design and testing of a pilot implementation of a mobile and web-based system for processing claims forms, based on two prior field visits to Uganda. Based on a comparative device study, semi-structured interviews, health clinic surveys, and a brief deployment, we affirm the selection of the mobile phone as a platform from the health clinic perspective, and further suggest that effective design for development requires more than addressing requirements of the the “users” of the mobile phones but also all the other stakeholders involved, who may have conflicting requirements.

According to the Wall Street Journal, “[Data produced by the use of mobile phones] generates immense commercial databases that reveal the ways we arrange ourselves into networks of power, money, love and trust.” As mobile phone use increases and applications become increasingly sophisticated, the volume of mobile data we create continues to grow at an incredible rate, creating new possibilities and posing new challenges to notions of privacy.

Businesses want this data for marketing. Congress wants to regulate it. Activists and privacy advocates want to ensure that it is not used to compromise their safety or freedoms. Meanwhile, projects such as UN Global Pulse want to use information gleaned from mobile phone use to detect and prevent slow-onset humanitarian crises. Non-profit organization mPowering is developing customized mobile apps to help reach the ultra poor -- people living on less than $2 a day -- and connect them with funding opportunities and programs in the developed world.

Reaching individuals and supplying resources in remote regions has huge challenges. The goal of mPowering is to leverage existing mobile infrastructure to open up channels of access. The organization has ongoing programs in Nepal and India which provide incentives to poor individuals for reporting to school or work, via mobile application. The organization is also working to create a mobile donor app to further connect the poor with funding opportunities.

Before a mobile app can be developed and deployed, the mPowering team conducts field research and partners with local institutions. We spoke with Kamael Ann Sugrim, Co-founder and CEO of mPowering, to find out how an app is developed.

Programs in Nepal and India

The mPowering organization is a year old and currently has two programs underway which utilize mobile apps. In Bhaktapur, Nepal, women earn points for reporting to work, and the points can be redeemed for food, clothing, and medicine.

In Orissa, India, 175 children in the village of Juanga earn points for attending school and can redeem the points for food, clothing, and medicine. Teachers have been supplied with donated Android phones with the mPowering application. Through the app, they can “scan” children in for attendance.

Before an mPowering application is developed, such as the one being used in Orissa, the team conducts field research in program locations. Also, parterning has proved key. mPowering employs local individuals to help operate the programs in Nepal and India. The organization also holds training sessions for families and individuals who receive mPowering phones, and supplies phone chargers at check-in locations.

What did not work? What were the challenges?: 

A recurring challenge for mPowering is mobile access and service. In some program locations, service can be limited and it can be difficult to figure out how to collect data via the app. There have been some design challenges, too, which stem from a push versus pull approach.

 


Who Cares Where I Am, Anyway? An Update on Mobile Phone Location Tracking

Apple’s release of version 4.3.3 of its iOS operating system “..kills iPhone tracking”, according to a recent article. After nearly three weeks of public attention on this issue, this news will perhaps appease some iPhone fans but is not likely to end the debate over what users should know and control about their smartphones’ location tracking abilities. Like Apple, Google’s Android and Microsoft’s Windows Phone systems have also recently come under fire, though important differences exist in the way each company collects and uses location-based information.

We have reviewed recent articles and research on each of these mobile operating systems’ location tracking capabilities and will describe the various claims made and the research undertaken to test these claims.

eMOCHA: Android Data Collection for mHealth

Using mobiles for data collection is increasingly common, particularly in the area of mobile health and with a focus on community health workers. eMOCHA is a program using a smartphone Android application for storing and transmitting data easily.

Developed by the Johns Hopkins Center for Clinical Global Health Education, eMOCHA (which stands for “Electronic Mobile Open-source Comprehensive Health Application”) uses video, audio, touchscreen quizzes, GPS and SMS to collect and analyze large amounts of data. Larry William Chang, director of field evaluations for eMOCHA, explains in an interview with MobileActive.org that the inspiration for developing the tool came out of researchers’ experiences in the field and their desire to build solutions to gaps in health care data collection systems.

Fellow team members include Miquel Sitjar, lead developer for eMOCHA, and Robert Bollinger, director of the Johns Hopkins Center for Clinical Global Health Education. Chang says, “We all had these public health problems and these education and training problems that we were looking to solve, and we wanted to build a mobile-based platform that could address all the different challenges we were facing. So we designed eMOCHA to address some of the public health and patient care problems that we were seeing with our other work.” eMOCHA’s first deployment began in October of 2010, and new pilots have been announced for 2011.

Lessons from m-Health Projects: The Tech is the Easy Part

Adherence reminders, patient data transmission via community health workers, HIV/AIDs info services – mobile phones can be used in a variety of health settings. As mobiles have become cheaper and more easily available around the world, mobile health projects have followed, taking advantage of the devices’ data storage capabilities, information transferring potential, and social networking features.

MobileActive has covered the m-health area extensively as NGOs, aid organizations, and governments continue to launch new projects incorporating ICTs into their work. Organizations like the Praekelt Foundation, which runs multiple mobile health projects, Pesinet, a micro-insurance and community health worker data collection tool, Dimagi, which developed CommCare (a project that helps community health workers promote healthy behaviors in patients), and MoTeCH, a Grameen Foundation project that uses mobiles to send medical advice to pregnant women and young parents along with creating a data managing resource for community health workers, are exploring the potential that mobile technology offers for delivering health care.

Looking at some of these organizations’ experiences, we put together a list of key lessons organizations are learning as they develop m-health projects:

A 2010 Country Governance and Anti-Corruption (CGAC)-funded pilot in Guatemala employed entry-level mobile phones in conjunction with EpiSurveyor, a free, web-based software for data collection, to drastically reduce cost, facilitate accuracy and accelerate implementation of a nationally-representative beneficiary survey of Guatemala‘s conditional cash transfer program.

As such, it illustrates the potential of mobile phone-based data collection to strengthen program monitoring, evaluation and implementation, in particular in remote and marginalized areas highly populated by indigenous peoples.

Txteagle in Flight: Mobile Data Collection for Disaster Preparedness

Txteagle is a data collection and engagement platform that leverages mobile airtime compensation for data collection and customer engagement. It is currently used by one nonprofit organization to survey constituents about disaster preparedness. We looked into the company and one of its customers.

Txteagle widely advertises its ability to reach 2.1 billion mobile subscribers currently. MobileActive.org spoke with Nathan Eagle, co-founder of the service, to learn more about how it works, how many active participants there are and where some of the large numbers come from. We also spoke with Terry Gibson, project manager for the Global Network of Civil Society Organisations for Disaster Reduction, who is currently using txteagle to collect mobile data on 40,000 respondents in 48 countries.

The txteagle platform is a hard technology to build, and there will be updates. Working with operators has been a challenge, as networks go down periodically.

Abstract: 

Many services taken for granted in the developed world are often missing from the developing countries. One typical example is that of map systems that form the basis of several location-driven services. Its heavy reliance on content, provides a huge barrier towards building such systems. Further, in developing countries like India, the infrastructure typically has a history of unplanned development, leading to unorganized addresses and absence of standard naming conventions for roads. Detailed map systems such as online maps have only recently started becoming available but for major cities. Remote towns and villages remain out of reach till date.

In this paper, we investigate a community-driven approach for creating maps in developing regions - following Web 2.0 principles, but not entirely relying on the existing Web. Our system, dubbed FOLKSOMAPS is an intelligent, community constructed map system, particularly architected with developing regions in mind. We present the design of FOLKSOMAPS, followed by an implementation of our proof-of-concept system. We present user studies aimed at understanding the uptake, usability and utility of FOLKSOMAPS. The results indicate a strong need for such a community-generated mapping ecosystem.

From January 9th to January 15th, Southern Sudan held a referendum to decide if the region should become independent from North. Although results have not yet officially been announced, estimates indicate that the referendum will pass with an overwhelming number of pro-independence votes. During this time, keeping citizens informed of new developments is crucial, and one of the best ways to reach large numbers of people is through the radio. The Sudan Radio Service, which has been operating since 2006, recently began incorporating mobile technology into its work to both monitor the reach of its broadcasts and to solicit reader feedback.

Roughly 13 percent of the world’s population still lacks access to a regular supply of clean drinking water, and monitoring current water pumps and sanitation points is an important part of making sure that areas that have gained access to clean water don’t lose it. Water for People is a non-profit organization that monitors water and sanitation points in the developing world; last February, the organization began to investigate how mobile technology could help their work and from this, FLOW was born.

FLOW (Field Level Operations Watch) is an open-source, Android application that allows field workers to use mobile phones to document how well water pumps and sanitation points in the developing world are functioning, then transmit that data to create an online tagged map of target regions.

Abstract: 

Still in its infancy, mHealth, the use of mobile technologies for health, runs the risk of not realizing its full potential due to small-scale implementations and pilot projects with limited reach. To help shed light on these issues, the mHealth Alliance commissioned an in-depth exploration of the policy barriers and research gaps facing mHealth. The review identified significant gaps in mHealth knowledge stemming from the limited scale and scope of mHealth implementation and evaluation, a policy environment that does not link health objectives and related metrics to available mHealth tools and systems, and little investment in cost-benefit studies to assess mHealth value and health outcomes research to assess success factors and weed out poor investments.

As illustrated throughout the literature, the current single-solution focus of mHealth needs to be replaced by using mHealth as an extension and integrator of underlying health information systems along the continuum of care. Creating a strong collaborative foundation will be instrumental in driving and positioning public and private investment in mHealth in a way that contributes to achieving improved access to health information and services, health outcomes, and efficiencies.

Within such structured paradigms, planning for infrastructure investment and human resources capacity strengthening and identifying public and private stakeholders within the ecosystem to take leadership for the development, testing, implementation, and evaluation of mHealth activities can be appropriately mapped and planned and mHealth services more effectively extended to support the health of citizens and the work of health professionals and administrators.
